Here is a basic configuration to get MAC auth functional using our radius services. 
aaa new-model

aaa authentication dot1x default group radius
aaa authorization network default group radius
aaa accounting update periodic 5

aaa authentication login default group radius local
Enable RADIUS: 
radius-server host rad01.vir.isptriage.com auth-port 1812 acct-port 1813 key YOURSECRET
radius-server retransmit 3
radius-server timeout 5
radius-server deadtime 10
Add company identifier 
hostname CompanyA-SW1
radius-server attribute 32 include-in-access-req
Per port auth 
interface GigabitEthernet1/0/1
 switchport mode access
 authentication enable
 mab
 authentication host-mode multi-auth
 authentication order mab
 authentication priority mab
 authentication port-control auto
 spanning-tree portfast
Enable port auth globally
